Faridabad, Police Station Shyampur, Dhaka-1204, Bangladesh, all are by faith Muslim, by Nationality Bangladeshi by birth. do hereby solemnly affirm and jointly declare as follows :

That we are the natural parents of MOSHAROF AHMED, date of birth 01/04/1985, holder of Bangladesh International Passport # X 0019647, son of Abul Basar & Meherun Nessa, of 60KA, Dhalka Nagar Lane, P.O.
