Subject : Regarding payment of monthly medical allowance of Mr. Showkat Ahmed Al Faroque, retired SPO from Sonali Bank, Zonal Office, Kishoregonj.

Dear Sir,

In the matter of above-mentioned subject matter the medical allowance card No.: 1262 of the said officer receive vide Ref. No.: ProKa/KeHiBi/KaTaSha/Ref./3496, dated : 30/10/2005 AD of Sonali bank, Central Accounts Department, Employees Funding Branch, Dhaka has been sent herewith to under take required steps.

The above mentioned officer/employee has taken direct/final retirement from bank  service on 17-09-05 AD willingly. He will not get the monthly pension.. But from the date of retirement viz from dated 17-09-05 AD he will get medical allowance by the amount of TK. 400/- per month for lifetime. So, the medical allowance for which the month while that month will be ended, in the subsequent month the Head Office should be debited with money. For this a card of medical allowance No.: 1262 has been enclosed herewith and sent to you. At the time of payment of allowance the enclosed card should be written with full signature every months. If any jeopardy is arisen in this regard the declaration No.-1170 of dated 30/04/98 AD of Head Office should be followed. The net pension of the concerned officer is TK. 7,200/- only will be applicable for assessing the festival bonus.
