AFFIDAVIT – DECLARATION

I, FARAH SHEERAZI, W/o. S. A. A. Sheerazi,  Address: Malabika Apartment, Flat # 6/B, 647, Bara Moghbazar, P.S. Ramna, Dhaka - 1217, Bangladesh,, by faith Muslim, by Profession - Business and by Nationality – Bangladeshi by birth, do hereby solemnly affirm and declare as follows:

가.                   That I am a permanent resident and citizen of Bangladesh by birth.

02.     That my maiden name was ROMANA HUQ CHOUDHURY and after my marriage my name has been changed as FARAH SHEERAZI. So, my name has been written in my Passport (No. O 0799350 (Old) B 0962888 (New)) as FARAH SHEERAZI.

나.                                           That ROMANA HUQ CHOUDHURY and FARAH SHEERAZI  is same person.

다.                                            That my date of birth has been erroneously written as 22/05/1978 instead of 22/05/1970 in my Secondary School Certificate documents.   

That the statements made above are true and correct to the best of my knowledge and belief, in truth whereof, I swear and sign this Affidavit – Declaration before the Notary Public, Dhaka, Bangladesh.
