AFFIDAVIT - DECLARATION

I, DULAL HOSSAIN PRAMANIK, S/o. Mazib Pramanik, Mother: Parvin Begum of   Vill. Rangachira, P.O. Kakua, P.S. Tangail Sadar, Dist. Tangail, Bangladesh, by faith Muslim, by Profession – Private Service and by Nationality  - Bangladeshi by birth, do hereby solemnly affirm and declare as follows:

1.       That I am a permanent resident and citizen of Bangladesh by birth.

2.       That my name has been erroneously written as "DULAL” in my Passport No. AC 3092899 instead of  " DULAL HOSSAIN PRAMANIK ".

3.       That my correct name will be as " DULAL HOSSAIN PRAMANIK " in my Passport as well as my other documents.

That the statements made above are true and correct to the best of my knowledge and belief, in truth whereof, I swear and sign this Affidavit - Declaration before the Notary Public, Dhaka, Bangladesh.
