AFFIDAVIT - RELATIONSHIP CERTIFICATE

I, MD. RAFIQUL HAQUE, Son of Late Haji Din Mohammad, Address: Vill. Zinzira, Howly, P.O. Keranigonj, P.S. Keranigonj, District - Dhaka, Bangladesh, by faith -  Muslim, by Profession - Business and by Nationality – Bangladeshi by birth, do hereby solemnly affirm and declare as follows:

1.       That I am a permanent resident and citizen of Bangladesh by birth. 

2.                 That I have no son and no daughter.

3.                 That the father of Md. Sazzad Hossain is my cousin.

4.                 That Md. Sazzad Hossain is my nephew and I am his uncle. I am  the sole guardian of my nephew as he stays with me in my family.

That the statements made above are true and correct to the best of my knowledge and belief, in truth whereof, I swear and sign this Affidavit – Relationship Certificate before the Notary Public, Dhaka, Bangladesh.
