No- AA7323177 and his wife Mashuka Begum Wife of Mr. Shamsuddin Md Iqbal

lives at Japan, address: Room No-308, Winbell Solo Kumagaya-3, 2-216-1, Hongku,
Kumagaya, Saitama, Japan bearing residence card no-WD49113615ED and residence
card no- WD82099862ED wants to adopt our son named A.S.M Idris and wants to
keep him in their custody and to take care, maintain and to educate, build him
up like their own son and as a proper person like his father and mother.
4. That I/We agreed with their proposal as loyal help full
person.
5. That if they do not take care our son property then they are
bound to return our son to us in good condition without any hesitation at any
time.
6. They are adopting my/our son and took our son in their
custody they will communicate with us time to time.
7. That Shamsuddin Md. Iqbal Chowdhury and Mashuka Begum can
take our son to abroad frequently without any prior permission seeking from us
and I/We have no objection regarding movement of un land and abroad.
8. That they who are adopting my son are not in any way
entitled to behave inhumanly with my son.
What have been stated above are true to
