I beg most respectfully to state
that I, the following signatory Md. Alam Bahar, son of Late Md. Habib Ullah,
Vill-Sanarbagh, P.O-Aminshapara, P.S-Sonaimuri, Dist- Noakhali, I do inform you
in this connection that yesterday on the last dated 07/08/05 at about noon
12:00, under this Police Station in the Agrabad area, there lost of my passport
which No. 0097477, the original copy of Visa and the Airport Ticket for going
to Kuwait in original and my Akama, the Ticket No. is 2094896080 317.
Therefore, I request you to make
a Diary in your Police Station on the ground of aforesaid and oblige there by.
