I, SHULI AKTER, Wife of Shah Alam Mia, Address: 310/3, East Goran, Post Office: Khilgaon , Police Station: Khilgaon, District: Dhaka, Bangladesh, do hereby solemnly affirm and declare as follows:

01.       That I am a permanent citizen of Bangladesh by Birth.

02.       That ABU BOKOR SIDDIK (SHAKIL) is my son whose date of birth is: 25.06.1995

03.        That previously in all documents/papers of his School the date of birth of my aforesaid son was written as 25.06.1993. Therefore, from now all documents and papers of my son should now be written as 25.06.1995

04.       That the actual and correct date of birth of my aforesaid son is 25.06.1995.
