Dear Sir,

I beg most respectfully to inform you that I, the following signatory Md. Alam Bahar, son of Late Md. Habib Ullah, Vill-Sanarpar Barabari, P.S-Sonaimuri, Dist- Noakhali, I do inform you that I was performed all the formalities for going to Kuwait after taking a service and I was also collected the Airport Ticket as per formality. But due to my un-carefulness, I have lost of my passport which No. 0097477, Original Visa Copy, Air Ticket going to Kuwait of Original copy which No. 2094896080317 and of my Akama which was all keeping with me. There have made a General Diary in the Sonaimuri Police Station under Chittagong district of which  no. 445, dated 08/08/05. I am keeping myself with much disheartened for the cause of lost of above of my documents. As the above fact, I have submitted of this application to your favour as I may go to Kuwait in the stipulated of period after investigation of my above information.

Therefore, I hope and pray that you will be enough to arrange for me as I may go to Kuwait through the hand works of you and also request you to arrange for not going of any people to Kuwait using of my above said Passport, Visa, Akama (Resident permit) and Air Ticket and if So, I will grateful to you.
