I, Haji Somsu Miah son of Konu Miah of Address: Village-Bagmoyna, Post Office-Ranigonj Bazar, Police Station-Jagannathpur, District-Sunamgonj, Bangladesh by faith Muslim, by Nationality- Bangladeshi by birth solemnly affirm and declare as follows:-

1.      That I am a permanent resident and Citizen of Bangladesh by Birth.

2.      That Amirul Haque is my nephew. I am his Uncle. He was born on 18/06/1968 (18^th June, 1968) at place of birth Sunamgonj.

That the statement made above is true and correct to the best of my knowledge and I have signed in this Affidavit.
