I, AFIA KHANOM CHOWDHURY wife  of Late Abdul Muhit Chowdhury of Permanent Address- Sonali-2, Mozomdar Para, Post Office- Sylhet, Police Station- Kotwali, Dist- Sylhet and Present Address: Village- Tarash, Post Office- Tarash , Police Station- Tarash , Dist- Sirajganj, Bangladesh, by faith Muslim by Nationality- Bangladeshi by Birth , do hereby solemnly affirm as follows:

1.         That I am permanent citizens & Nationality of Bangladesh and Bangladeshi by birth.

2.         That MOSTAL AHMED CHOWDHURY , Date of Birth : 25-05-1977 son of Late Abdul Muhit Chowdhury  & Afia Khanom Chowdhury of Permanent Address- Sonali-2, Mozomdar Para, Post Office- Sylhet, Police Station- Kotwali, Dist- Sylhet and Present Address: Village- Tarash, Post Office- Tarash , Police Station- Tarash , Dist- Sirajganj, Bangladesh is my son holder of Bangladesh International Passport # E 1550883 issued at Sirajganj , Bangladesh , on 21^st January 2010.

3.         That my actual & legal beloved younger brother MD. MANIK date of birth 16/09/1968 holder of Bangladeshi Passport No. Q- 0535069 now is living in South Africa.

4.         That my aforesaid brother is still UN-MARRIED and I have no objection if he marriage any girl in abroad.
