Shyampur, Dhaka-1204, Bangladesh, all are by faith Muslim, by Nationality
Bangladeshi by birth. do hereby solemnly affirm and jointly declare as follows

National of Bangladesh and Bangladeshi by birth.

MOSHAROF AHMED, date of birth 01/04/1985, holder of Bangladesh International
