          I would like to inform you  that I Mizanur Rahman, Card No. 398, Passport No. N  0714375, at present  I have been suffering from the disease, as per advise of the Physician I should to take bed rest. The validity of my visa to be expired on 24/06/2001.

So, therefore, I pray and hope  to please take necessary step for extension of validity of my Visa for the period of two months. Please  reply.
