I beg most respectfully to inform you that I, SHELA TALUKDER, wife of Ershad Talukder, Mother: Shajeda, Address : Jatrari, P.O. Haiderpur, P.S. Muktagasa, Dist. Mymensingh on last 15^th December, 2009AD. I was going to a Travel Agency at Motijheel and on the way at Dainik Bangla under P.S. Motijheel the Passport issued in my name was lost The Passport No. E 1830375, Date of Issue: 01/03/2009, Place of Issue : Dhaka.  

Therefore, I hope and pray that you will be kind enough to entry a General Diary regarding lost of my Passport and oblige thereby.
