Dear Sir,

I beg most respectfully to inform you that I am MAHMUD HASAN (32), son of Late Amzad Hossain, Vill. Pachroha, P.S. Kapasia, Dist. Gazipur hereby inform about lost that on 9/4/12 at about 10 AM. my brother SAIFUL, Date of Birth 11/7/83, Father: Amzad Hossain, Mother: Anwara Begum of Vill. Pachroha, P.S. Kapasia, Dist. Gazipur, his original Passport No. X0444464, Issue Office – Dhaka, Date of Issue – 6/6/2005 was going to Dhaka for the purpose of its renewal and on the way when I reached to Kapasia Bus Stand and at the time of getting into the bus and the Bus was full of passengers and from the pocket of my Panjabi the Passport was lost. It was not found after very searching. The searching of passport is still continuing. The reporting is delayed due to busy for searching the Passport.   

Therefore, I hope and pray that you would be kind enough to make a General Diary about above losing matter for the future and oblige thereby.
