Subject:   Notice of Talaq -e- Tafwiz given by the wife to the husband as per Sub - Section 1 of the Section 7 of the Ordinance No. 8 of Muslim Family Act, 1961.

                             I would like to inform you that I married with Md. Saifur Rahman Khan S/o. Mr. Late Abdul Bari Khan of Vill.  Kalihati, P.O. Kalihati, P.S. Kalihati, Dist. Tangail  through a Registered Marriage Deed (Nikah Nama).

After marriage my husband has violated the terms & conditions of the Nikah Nama, so, having no means today I have accepted myself Talaq -e-  Tafwiz  by registered at the Kazi Office, Ward No. 13, City Corporation of Dhaka District being empowered for Talaq -e- Tafwiz.
